Types of reports dictated by a hospital or medical center History and Physical Examination The History and Physical Examination (H&P) is generated shortly before or after a patient is admitted in the hospital. This report usually contains - Chief complaint or presenting problem. - History of present illness which outlines the events leading to the patient's hospitalization. - Past medical history which lists medical and surgical problems from childhood to the present, medications, and allergies. - Family History, which outlines the medical condition of parents and other family members. - Social history, which describes the patient's occupation, lifestyle, and habits. - Review of systems, which is the medical condition of the patient's major organs. - Physical examination. |
